Notice of Privacy Practices
Your privacy matters to us.
This notice describes how medical information about you may be used and disclosed, and how you can access that information. Please review it carefully.
Last updated: January 2026
At Great Lakes Eye Care, protecting the privacy of your health information is part of caring for you. We are required by law to maintain the privacy of your protected health information (PHI), to provide you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How we may use and share your information
We use and disclose your health information only as needed to care for you and run our practice responsibly. The most common reasons include:
- Treatment — sharing information among your doctors, technicians, and other providers involved in your care, including coordinating with your referring physician or other specialists.
- Payment — billing and collecting payment from you, your insurance company, or another third party for the services we provide.
- Health care operations — quality review, training, scheduling, and the everyday activities that keep our practice running.
- Appointment reminders & follow-up — contacting you about appointments, test results, or treatment alternatives that may interest you.
Disclosures that require your written authorization
Most uses and disclosures beyond those above — including most sharing of psychotherapy notes, uses for marketing, and any sale of your information — require your written authorization. You may revoke that authorization, in writing, at any time, except to the extent we've already acted on it.
Your rights regarding your health information
You have the right to:
- Access and copy your medical and billing records. See our Records Release page to make a request.
- Request corrections to information you believe is incomplete or inaccurate.
- Request confidential communications — for example, asking us to contact you at a specific phone number or address.
- Request restrictions on certain uses and disclosures of your information.
- Receive a list of certain disclosures we've made of your information.
- Receive a paper copy of this notice, even if you agreed to receive it electronically.
- Be notified if a breach occurs that may have compromised the privacy of your information.
Our responsibilities
We are required to protect the privacy of your information, to notify you promptly if a breach occurs, and to follow the duties and privacy practices described in this notice. We will not use or share your information other than as described here unless you tell us we may, in writing.
Changes to this notice
We may change the terms of this notice at any time, and changes will apply to all information we hold. Any revised notice will be posted here and available in our offices upon request.
Website privacy
Information you submit through this website — such as an appointment request or contact message — is used only to respond to your request and is not sold. Please do not include sensitive medical details in online forms. For anything urgent or confidential, call our office directly.
